Students May Be Solution to Shortage of Medical Professionals

$2.4 Million Grant from Kaiser Permanente places students in pharmacy, dental medicine and medicine in rural Colorado.

Colorado is facing a shortage of medical professionals, especially in rural communities, causing some people to forgo preventive care or treatment for serious illnesses. Kaiser Permanente is now teaming up with the University of Colorado Denver’s health sciences programs to provide a unique solution to the shortage by training more medical professionals interested in rural health practices.

As part of its community benefit program, Kaiser Permanente made a $2.4 million dollar grant to establish the University of Colorado Denver's Interdisciplinary Rural Training and Service Program (IRTS).

"I applaud the University of Colorado Denver and Kaiser Permanente, two local health care leaders, for pushing the envelope and thinking innovatively about the type of public-private partnership that can address critical health needs in our state," said Gov. Bill Ritter.

The IRTS program will build upon successful but independent programs in the School of Medicine, the School of Dental Medicine and the School of Pharmacy. It will combine the disciplines so students practice together, gaining a greater appreciation for the way their specialties can collaborate to improve patient care. The ultimate goal of the IRTS is to apply an integrated training approach in order to increase the number of students and enhance access to care in Colorado's Delta, Garfield, Routt, Jackson and other rural counties.

"We know first-hand that patients are happier with their care, physicians are more satisfied, and medical outcomes are improved when care is delivered in a coordinated fashion," said Jandel Allen-Davis, MD, vice president of government and external affairs for Kaiser Permanente Colorado. "The beauty of this partnership between Kaiser Permanente and the University of Colorado Denver is that it aims to achieve these success points while focusing the resources where they’re needed the most – in our rural communities."

Mark Deutchman, MD, professor of family medicine, will lead the IRTS and the Colorado Area Health Education Center (AHEC) will serve as the administrator and evaluator of the program. The different disciplines will interact in the following ways:
 School of Pharmacy: Students will focus on improving the health of patients with heart disease, including diabetes, high blood pressure, blood clotting and high cholesterol and other lipid disorders; and provide multidisciplinary clinical education in rural communities.  School of Dental Medicine: Students will increase the number of rural communities served by the Colorado Smilemakers Mobile Dental Clinic, which provides screenings and basic dental treatment. The students will be encouraged to practice in rural areas upon graduation.  School of Medicine: The successful program, which takes students into rural communities to shadow and train along side medical professional in hospitals and clinics, will expand to include shadowing of dentists and pharmacists currently practicing in rural communities.

The numbers demonstrate the need. According to the Colorado Rural Health Center, seven counties have no dentists, six have no full time primary care physicians, and one has no primary care physician.

"Colorado is largely a rural state and many of our rural counties are facing significant shortages in health care professionals living and working in their communities," said Ned Calonge, chief medical officer of the Colorado Department of Public Health and Environment. "This innovative partnership will help address these shortages by supporting the recruitment and retention of new health profession graduates in currently underserved communities."

Building upon already successful programs gives Kaiser Permanente's IRTS grant a strong starting point. The School of Medicine's Rural Track was founded four years ago and aims to increase the number of students who eventually enter, and remain, in practice in rural areas of Colorado. For the past 10 years, the School of Pharmacy has sent its students out for six-week, full time learning experiences at pharmacies throughout the state. The School of Dental Medicine's Smilemakers Mobile Dental Clinic has been providing care to rural Coloradans for more than two years, creating rural practice opportunities for students in the dental hygiene, dental and international programs. What is unique about the Kaiser Permanente grant is the integrated approach for the different disciplines to deliver the care side by side.

"Rural track is a critical part of a long pipeline," said Mark Deutchman, MD, director of the Rural Track. "That pipeline starts by getting rural young people thinking about health care careers in general during their early years, getting them properly prepared for college, getting them into professional schools and supporting them in returning to rural communities to live happily and be successful in their work. Kaiser Permanente's IRTS grant is exactly the kind of help we need." 

The IRTS grant is part of a combined $3 million grant from Kaiser Permanente Colorado to the University of Colorado Denver’s health sciences programs. The Colorado School of Public Health received $600,000 to support the creation of the Center for Public Health Practice, which will focus on improving the health of communities and individuals throughout Colorado. The grant was facilitated by the University of Colorado Foundation.

According to SDI, about half of patients beginning therapy on popular osteoporosis drugs discontinue their treatment within 6 months. Amgen’s Prolia is the newest product to enter the market. Prolia differs from existing therapies as it is given every 6 months via injection.

According to a recent analysis by SDI (http://www. sdihealth. com), a leading healthcare market insight and analytics firm, about half of patients beginning therapy on popular osteoporosis drugs discontinue their treatment within 6 months. Amgen’s Prolia, which was recently approved for postmenopausal women with osteoporosis at high risk of fracture, or patients who could not tolerate other osteoporosis therapies, is the newest product to enter the market. Prolia differs from existing therapies as it is given every 6 months via injection.

Currently, alendronate, sometimes filled as Fosamax but more frequently as the generic equivalent, is the most popular treatment for osteoporosis. In the 12 months ending April 2010, alendronate accounted for almost 24% of drug issuances for the condition.

Generic alendronate is most often dispensed as a once-weekly therapy. SDI found that 49% of new alendronate patients had discontinued the drug 6 months later. Another 3% had switched to another therapy. After 12 months, 61% of new alendronate patients had discontinued therapy. 

The weekly form of Actonel had similar discontinuation rates. Fifty-four percent of patients taking Actonel once a week had discontinued therapy 6 months after starting; 64% had discontinued after 12 months.

The once-a-month version of Actonel fared slightly better after 6 months with only 47% of patients having discontinued therapy. But at 12 months, 63% had discontinued. The oral form of Boniva, also used once a month, had an even higher percentage of patients discontinuing therapy – 57% after 6 months and 70% after 12.

“Drug adherence is an issue that many in the healthcare industry struggle with, including physicians, pharmacies, and payers. If patients choose to discontinue therapy, they may be at a greater risk for progression of their disease or condition,” said SDI Chief Medical Officer Gregory Hess, M. D., M. B.A.. “There are financial ramifications as well. Using drug therapy to treat osteoporosis is less expensive than treating a patient with a broken bone because they didn’t adhere to therapy.”

The interior of North Fork Bank’s newest branch in Flower Hill, NY, hearkens back to another era.

The branch, recently opened on Northern Boulevard on Long Island’s prestigious Gold Coast, occupies 4,500 square feet in a new luxury retail center just a half mile east of such designer shops as Gucci, Tiffany, Louis Vuitton, Yves St. Laurent and Ralph Lauren at the Americana Shopping Center on Manhasset’s Miracle Mile.

Designed by Mineola, NY-based JRS Architect, PC to be one of North Fork Bank’s premier Long Island locations, the elaborately detailed banking center recalls “the historic richness of the community and the look and feel of the village,” said Alexander Hadjiyane, senior vice president of the firm, noting that the area’s historic homes and the Roslyn Clock Tower provided cues for the design. “We wanted to have a rich, elegant, hand-crafted look.”

The new branch is the latest in a string of projects in JRS Architect’s growing portfolio of bank branch designs for North Fork and other financial institutions, among them JP Morgan Chase, New York Community Bancorp’s Roslyn Savings Bank and Bethpage Federal Credit Union.

The new North Fork branch’s classical design with its columns and pedestals reflected the architects’ desire to make the bank appear as an established fixture of the community. “We went back in time,” said Ray Maggiore, project manager. “But it’s not a complete throw back to what banks were.” A flat panel television screen on one wall emphasizes the point.

With its terrazzo floor radiating in a starburst from a central checking desk; deep brown, palm-leaf pattern carpet; woven dark wood fixtures; metal painted ceiling tiles; hand-crafted burnished brass chandelier and oversized plush leather chairs, the new bank has an other-worldly look, resembling the lobby of a luxury resort hotel on a tropical island.

The finishes, millwork and lighting futures designed by the firm were influenced by the Arts and Crafts movement popular in the late 19th Century.

Customers appear to like what they see. “They are completely surprised at the look,” said Hadjiyane, noting that bank employees also appreciate the design. “It made them feel very exclusive.”

Approximately 10 million Americans suffer from Age-related Macular Degeneration (AMD), a devastating eye disease and a leading cause of vision loss in people 65 years or older in the United States. Although there is no cure for AMD, recent studies show that eating foods rich in antioxidants such as lutein, zeaxanthin, omega-3 fatty acids, beta carotene, vitamin C, vitamin E and zinc, may reduce the risk of AMD, or slow its progression in some people.

So, what type of foods should you eat?
Most fruits and vegetables contain Vitamin C, including oranges, grapefruit, strawberries, papaya, green peppers and tomatoes. Vitamin E can be found in vegetable oils (safflower and corn oil), almonds, pecans, wheat germ and sunflower seeds. For beta-carotene, try deep orange or yellow fruits and vegetables such as cantaloupe, mangos, apricots, peaches, sweet potatoes and carrots. Dark green leafy vegetables such as spinach, kale, collard greens, broccoli, and asparagus are the primary sources of lutein and zeaxanthin. Good sources of zinc include beef, pork, lamb, oysters, eggs, shellfish, milk, peanuts, whole grains and wheat germ. Good sources of omega-3 fatty acids are leafy green vegetables, nuts, fish, and vegetable oils such as canola, soy, and especially flaxseed. Important Issue at Crux of Environment, Drug Regulations, Child/Public Safety and Healthcare Reform

S. Senate Special Committee on Aging during a hearing on drug waste and disposal. Hendrickson’s testimony transcript and video of the hearing are available on the U. S. Senate Web site. Chairman Kohl (D-WI), along with Senators Corker (R-TN), Collins (R-ME) and Casey (D-PA), attended the hearing. The majority of the witnesses asked for changes to the Controlled Substance Act to allow non-DEA registrants the ability to accept controlled substances through take-back programs. The DEA is on record supporting H. R. 1359 which would give the DEA this authority.

Covered in the testimony is the intersection between drug regulations, environmental concerns and the human tragedy associated with overdoses on prescribed drugs found in medicine cabinets, a human health issue that could have implications within broader healthcare reform initiatives. The panel statements included a drug enforcement official who indicated that a nationwide drug take-back initiative is in the planning stages; a parent who lost a son due to the use of an unused prescription medication left in a medicine cabinet; and among others, Stevan Gressitt, Founding Director, Maine Institute for Safe Medicine, Faculty Associate University of Maine, Center on Aging, who discussed Maine’s statewide program allowing consumers to return unused drugs back to the DEA through the U. S. Postal Service – the program has recovered one ton of unused pharmaceuticals since 2007, saving the state $250,000 per year in associated costs.

Covered in Mary Hendrickson’s statement were the following:
 The environmental and misuse/abuse concerns associated with improper disposal or non-disposal of medications, as often practiced by long term care facilities and in the home.  The simple yet highly regulated process of reverse distribution – to waste-to-energy facilities – of unsold medications from pharmacies, of which GENCO Pharmaceutical Services is the leading provider.  An overview of the GENCO take-back program in Wisconsin, which allowed for consumers to return unused medications for incineration in a waste-to-energy facility, meeting with great success from both public safety and environmental standpoints. During the six month program, 1,259 households returned 15,164 medications.  The process for handling pharmaceutical waste: Incineration of drugs provides an alternative to burning coal for electricity, resulting in a net greenhouse gas reduction, while keeping drugs out of landfills and the water supply. A 2002 U. S. Geological Survey study of 139 bodies of water across the country found that over 80 percent of the water samples were contaminated by prescription drugs, which have been shown to harm fish and wildlife.  The barrier to entry for GENCO into a consumer take-back market, despite the company’s standing as the leading provider of reverse distribution for the pharmaceutical industry’s overstocked and expired drugs. The Controlled Substance Act allows only for transfer of controlled substances among the DEA registrants. When a pharmacy dispenses the medication to a patient, the patient is not considered to be within the closed loop of distribution and is not a DEA registrant. As a result, reverse distributors as well as the other DEA registrants cannot take controlled substance product back from patients.

Other experts provided testimony presenting the opinion that the environmental, cost and safety benefits of take-back programs far outweigh the minimal risks of criminal diversion of controlled substances into a black market.

As the leader in pharmaceutical returns for industry, GENCO is well-poised to manage the logistics of a nationwide take-back program. Government’s healthcare reform initiatives may point the nation in the direction of improved data collection regarding the costs associated with over-prescribing, the human tragedy of overdoses associated with the medicine cabinet and the environmental issues with poor disposal – including the issue of medication seepage into the water supply. Legislation reform to address the legislation impeding drug take-back programs is expected over the next two years.

S. and Globally.'' HIV prevalence rates are highest among the most marginalized populations in both the US and abroad, whether they are the poor, those living in rural or indigenous communities, men who have sex with men, people of color, injection drug users, or women.

According to UNAIDS, women and young girls presently comprise more than 60% of those who are living with HIV in sub-Saharan Africa - and the numbers continue to grow. Any solution to the epidemic must acknowledge the devastating gender dynamics of AIDS and place women's empowerment at the center of programming and planning.

"The US government stands at a pivotal moment in the history of HIV/AIDS and global health policy. The reauthorization of PEPFAR offers hopeful promise, if Washington commits to full funding," stated Jim Yong Kim. "Globally, it is essential that AIDS prevention and treatment are provided in the context of primary care including advanced care for tuberculosis and other opportunistic infections, screening and treatment of sexually transmitted infections and an emphasis on women's health."

PEPFAR reauthorization is fully appropriated by Congress, is targeted to prevent 12 million HIV infections and treat three million people over five years.

Sugar, sugar everywhere...
The facts Dr. Dean presents are hard to swallow. She relates that a staggering 1,000% more children today are diagnosed with diabetes compared to 1990, and that “…over 80% of type 2 diabetes is related to being overweight.” According to Dr. Dean, the Institute of Medicine (IOM) recommendations sanction up to 25% of calories in one’s daily diet from added sugar, amounting to approximately 33 teaspoons of sugar in a daily diet of 2,000 calories. However, the USDA states that the average annual sugar consumption per person has increased from 123.3 pounds in 1970 to 170.5 pounds in 2008, amounting to 50.5 teaspoons of sugar a day, exceeding the IOM limits by over 17 teaspoons every day.

Health risks associated with sugar consumption are legendary: hypoglycemia, adrenal stress, insulin resistance leading to diabetes, tooth decay, candida (yeast) and behavior disorders; sugar can also precipitate hypertension and high cholesterol in later life. So who gains from this dwindling spiral? As Dr. Dean points out in Kids’ Health, the refined sugar industry cultivates millions of acres of corn, sugarcane and beets annually. Companies that produce artificial sweeteners and additives also have a big stake in the game, as does the fast food industry, as do the advertising media, as do the pharmaceutical companies that manufacture insulin, drugs to treat ADHD, Prozac and other psychiatric drugs and medicines, as do the hospitals that treat those who have become too frail as a result of this vicious cycle.

The role of diet in ADHD...
How do children behave when certain foods are eliminated from their diets? In Kids’ Health Dr. Dean cites several studies that found striking improvements in mood, learning ability and social skills among public school students, children diagnosed with hyperactivity, ADD and other behavioral problems, and even juvenile delinquents. In these studies, children were taken off sugar, high-carbohydrate junk foods, foods with additives and common foods such as milk, eggs, wheat and citrus. Dr. Dean’s own patients, including children with ADHD and autism, have improved when they stopped consuming sugar, gluten, casein, soy, MSG and artificial sweeteners.

The booklet steps mightily into a current arena of controversy, challenging the medical and pharmaceutical establishment’s penchant for medicating children with powerful psychiatric drugs. Dr. Dean writes: “It has been said of conventional medicine that ‘when all you have is a hammer, everything looks like a nail.’ When doctors are allowed to treat ADHD with drugs without looking at the greater picture of diet and supplements, our children become the nails. American employers provide lots of benefits to their workers, but helping them to cope with the rising cost of driving to work by encouraging telecommuting hasnÂ’t been one of them, based on the results of an online poll at HR. BLR. com.

HR. BLR. com, a website for HR professionals, posed this question to site visitors between June 29 and July 6: “What, if anything, are you doing to help employees deal with high gas prices?”

HereÂ’s how the 499 participants answered:

Encouraging more carpooling: 4% Encouraging greater use of mass transit: 2% Allowing more telecommuting: 1% Some combination of the above: 6% Nothing: 86%
