Centegra to Offer Rehabilitation Services at Fox Point in McHenry
Outpatient facility will offer treatment options for residents and others

Centegra Health System announced today it will expand its rehabilitation network by opening an outpatient facility Nov. 1 at Fox Point, an independent and assisted living residence in McHenry operated by CRL Senior Living Communities. The clinic will provide a convenient, comprehensive treatment option for residents at Fox Point and its adjoining memory care community, Fox Point Manor.
“We are pleased to announce we will provide outpatient physical therapy, speech therapy and occupational therapy to the residents at Fox Point,” said Rachel Sebastian, vice president of service line operations for Centegra Health System. “We will provide continuous screening of the residents and their functional capabilities, allowing for an established baseline and the ability to identify their individual needs.”
All Fox Point residents will be screened following any inpatient hospital stay or when a change in function is identified by staff or family members.
The clinic also will be open to area residents who don’t live at Fox Point but are looking for an accessible location to receive rehabilitation services. The clinic at 3300 Charles J. Miller Road, will be overseen by Denise Dumont-Meyer, manager of outpatient therapy operations for Centegra Health System.
