Baptist Medical Center's GI Lab has acquired the HALO 360 Ablation Therapy System, a device that provides tightly controlled ablation to circumferentially remove diseased esophageal lining referred to as Barrett's Esophagus (BE). The procedure involves a standard endoscopy technique under conscious sedation. BE is a pre-cancerous condition associated with chronic gastroesophageal reflux, or GERD.

Gastroenterologists Ronald P. Kotfila, MD; Makau P. Lee, MD; Paul B. Milner, MD; and Shawn W. Panzer, MD, are trained in the use of the device. According to Dr. Kotfila, the procedure usually takes 30 to 60 minutes including the time to sedate the patient. "It is very safe," he notes. "It only burns a 3mm depth of tissue and no more. This is just enough to get the Barrett's lining without burning into deeper tissue."

Traditionally, treatment for Barrett's esophagus has been careful surveillance, an option that has been less than optimal. "Most gastroenterologists I have known feel that watching BE is like watching a colon polyp," says Dr. Panzer. "All adenomatous colon polyps are dysplastic by definition, and thus have malignant potential. No one watches colon polyps and then removes them only if they progress. In Barrett's, that progression could be directly to cancer."

While ablation of the diseased tissue provides a more proactive treatment option, it has not been widely used due to the limitations of available technology to treat the disease without harming normal tissue. "The traditional ablative approaches--electrocautery, argon plasma coagulation, photodynamic therapy--have shown an inability to completely ablate all of the pathological cells or caused significant postprocedure complications, such as perforation, strictures, photosensitivity, mediastinitis, etc.," notes Dr. Panzer.

"Previous endoscopic treatments made the esophagus lining look normal, but there were problems with Barrett's tissue 'hiding' under the 'normal' lining," Dr. Kotfila added. "Also, treatments for acid reflux disease such as medication and Nissen fundoplication have not been proven to eradicate Barrett's esophagus in most patients. So far, the HALO 360 is completely eradicating the Barrett's tissue. Long-term studies are still in progress."

The HALO 360 System makes broad use of endoscopic ablation of the esophagus practical by providing uniform and controlled ablation at a consistent depth. During the procedure, a sizing balloon is first used to size the esophagus. A correctly sized ablation catheter is then inflated within the area of the Barrett's. The energy generator is activated to deliver a burst of ablative energy that lasts less than one second. This removes a very thin layer of the diseased esophagus. New, healthy tissue replaces the ablated Barrett's tissue in three to four weeks for most patients. Following ablation therapy, patients resume acid suppression therapy.

"Over 5000 post treatment biopsies have found no buried glands under the regenerated squamous mucosa. The procedure is FDA-approved and is promising," said Dr. Panzer. "This approach is an extension of current technology and makes sense in regard to our approach to malignancy elsewhere in the body--colon, cervix, etc. The fact that it is minimally invasive, apparently effective, and a form of secondary prevention makes it one which could be potentially applied to all patients with BE. I plan to discuss and encourage this option with all of my patients who have Barrett's esophagus, based on current knowledge," Dr. Panzer concluded.

HCL Thames Medics, the National Framework agency for recruitment of doctors to the NHS, and part of HCL plc, said today that greater use of flexible and locum doctors is vital for dealing with the pressures on frontline staffing brought about by new EU rules, which put a cap on junior doctors’ working hours.

HCL Thames Medics was responding to research by the Royal College of Physicians which said that some hospital doctors now have to care for up to 400 patients at night because of the impact of the European Working Time Directive, which cut junior doctors’ working hours to 48 per week. This has had a dramatic impact on staffing levels in hospital wards.

The solution for Medical Staffing Officers is to increase the proportion of locum doctors in their frontline staff, thus creating a flexible workforce which could be staffed up and down quickly to meet fluctuating demand, the Company said.

Kirsten Simcoe, Managing Director of HCL Thames Medics, said that this would be more cost effective than increasing permanent recruitment of doctors as locums are only paid for the hours that they work so would not incur added hidden costs such as holiday and pension entitlement.

Simcoe explained: “With tougher times ahead for the public sector, the challenge is maintaining doctors’ staffing levels in a cost efficient manner. The need to do this is especially acute given the impact of the European Working Time Directive on junior doctors. More and more of our NHS clients are increasing the use of locum doctors in their workforce as this is a flexible and cost effective alternative to a frontline made up entirely of doctors permanently employed by the public sector.”

She concluded: “Such a low doctors to patient ratio as suggested in the report is worrying, and clearly unacceptable for both patients and medical staff. The solution is to create and maintain a flexible doctors workforce, to deliver the best possible care for patients at a cost that is acceptable to the NHS and the taxpayer.”

Investment in Infrastructure on University Avenue Attracts Business

When French Packaging Services owner, Warren French, was searching for a new Massachusetts location for his business, the town of Norwood turned out to be the perfect fit. After searching all over the Greater Boston area–from as far south as Canton to the northern city of Woburn–French found Norwood's business climate to be superior.

“Norwood really has it all,” French said. “From the tax rates, to a great local electric company to outstanding infrastructure.”

After doing business in Auberndale–one of the 13 villages of Newton, Massachusetts–since 1980, French wanted to expand his company. He also wanted to be part of what he says is a burgeoning southern expansion.

“There was a big northern expansion for a while and everyone started going up along Route 3,” French said. “Now, rates have risen in those areas, so companies are looking further down south at places like Fall River and Attleboro. Our position in Norwood puts us right on the head of the spear for that whole southern expansion.”

Located approximately 15 miles from downtown Boston, Norwood is a small town with big appeal for commercial and industrial property owners and tenants. The town is comprised of 44 percent commercial property, a figure that reflects the Norwood Board of Selectmen's willingness to welcome new businesses and provide a slew of incentives. Jerry Kelleher, Chairman of the Norwood Board of Selectmen, said the town's low tax rate is due to its “outstanding commercial property owners.”

“We also have our own municipal light department, our own electric department and three commuter rail stops,” Kelleher sad. “One of our goals is to help businesses find a home in Norwood.”

To that end, the town works closely with business owners to get them up and running. When French Packaging Services, which will be operational in Norwood by year's end, first began working with the town of Norwood, Warren French experienced immediate results.

“Steve Costello, the business ombudsman and town planner, has been with me since day one of the move, telling me who to contact and how to get things done,” French said.

One of French's neighbors in Norwood is a large-scale, 449,555 - square-foot, three-building distribution center on 55 acres at 625 University Avenue. Located just 200 yards away from French Packaging's 30,000 feet of leased space on Oceana Way, the property at 625 University Avenue is partly owned by Hackman Capital Partners, LLC, a private real estate investment firm that has acquired or developed more than a billion dollars of property over the past 24 years. Michael Hackman, founder and chief executive officer of Hackman Capital, said the vacancy rates in Norwood are falling fast due to the town's myriad attractive qualities.

“Norwood is racking up commercial tenants even in these difficult economic times,” Hackman said. “Our property on University Avenue has become even more appealing in recent years, as $2 million have been spent improving University Avenue between Route 1 and Canton Street. The plantings, granite curbing and resurfacing are all perks for us and our tenants.”

Kelleher said the improvement project was funded by a Massachusetts Public Works economic development grant.

“In order to attract top notch businesses you have to have the infrastructure,” Kelleher said. “This grant allowed us to fix the roads and ensure easy, smooth access.”

With top-notch employers like UPS, Analog Devices, and Mercer Human Resources Consulting, Norwood's unemployment rates dropped in recent months from 8 percent to 7.1 percent, according to the Massachusetts Executive Office of Labor and Workforce Development. And the local coffers are stable, according to Kelleher, who said the town has a healthy stabilization fund. Kelleher also said that Norwood is one of the few Massachusetts towns that has not had to override Proposition 2 ½, a 1982 measure that capped property tax hikes at 2.5 percent.

“A lot of towns have had to override that measure for operational purposes, but Norwood has been able to keep the measure in place,” Kelleher said.

Not only is the business climate booming in Norwood, but education in the area is reaching new highs as well. A new public high school will open in Norwood next year, making the town the first in the state to have a school designed and built under the Massachusetts State Model School Program. The $53.5 million high school was approved by 84 percent of the town's residents, according to Courtney Rau, a member of the Norwood Public Schools Committee.

“We are getting fifty-nine percent reimbursement from the state of Massachusetts to build this brand new, state-of-the art high school in Norwood,” Rau said. “The residents have come out in droves to support it.”
