African-American Heart Attack Survivors Cite Experience as a "Wake-Up Call" to Address Health and Life Issues

National Medical Association Survey of Heart Attack Survivors uncovers emotional impact, educational needs and disparity among racial groups.

African-Americans who have suffered a heart attack consider their experience a "wake-up call" that leads to reevaluating priorities, growing closer to faith and recognizing the importance of strong heart health behaviors, according to new survey findings announced today by the National Medical Association (NMA). While a majority of those surveyed view their heart attack as a second chance at life, the results surprisingly indicate that nearly 30 percent of African-Americans state they are not doing everything they can to avoid another heart attack. In fact, according to the survey findings, 27 percent of African-Americans do not take their heart medications (http://www. heartfeltsupport. com) exactly as prescribed by their physicians after their heart attack.

Approximately half of the heart attack survivors surveyed report there is not enough information available about preventing future heart attacks or what to do after having one. Many of those polled feel that speaking to another heart attack survivor could provide much needed information. Overall, the survey finds that while African-American heart attack survivors look at their heart attack as a wake-up call, they lack information to prevent a second incident.

"When we look at the percentage of the African-American population that suffer heart attacks (http://www. heartfeltsupport. com) and the resulting death rate, when compared to other ethnic groups, we see a clear disparity which illustrates a significant need for education and support initiatives for heart health (http://www. heartfeltsupport. com) within our community," stated Albert W. Morris, Jr., M. D., president of the NMA. "The National Medical Association through the 'Heartfelt Wake-Up Call' campaign hopes to create a community amongst survivors and provide additional information to these survivors and to embrace and support their efforts to live a heart healthy life after a heart attack and to prevent another from occurring."

To raise awareness in the African-American community, the NMA commissioned the survey of African-American heart attack survivors. The survey was conducted by Yankelovich, Inc. and funded and assisted by GlaxoSmithKline. The initiative is part of the "Heartfelt Wake-Up Call" campaign, started in 2005 by Mended Hearts, a heart patient organization affiliated with the American Heart Association. The campaign offers information to better support African-American heart attack survivors and their caregivers cope with life after a heart attack. Inside the Heart of a Survivor

The survey of more than 500 African-American heart attack survivors taps into the emotional impacts of heart attacks. A majority indicate their heart attack forced them to face their mortality (62 percent), they now spend more time with friends and family (66 percent), are motivated to accomplish goals (62 percent), and are trying to move closer to God and their faith (67 percent). A majority of heart attack survivors also say the experience made them realize how much they want to see their children and grandchildren grow up (68 percent).

The survey also shows that in addition to viewing their heart attack as a wake-up call (68 percent), those surveyed also acknowledge that they are at a higher risk for having another heart attack (93 percent), and that they are at an increased risk of developing a chronic condition such as heart failure (73 percent).

"Studies show that African-American heart patients may have a different natural history of heart failure than non-African-American patients and may be less likely to receive guideline-recommended, evidence-based therapies due to less access to care. Certain heart medications, like beta-blockers, are often underutilized even though they've been shown to reduce the chance of another heart attack and reduce the risk of dying in heart attack patients with damaged hearts," stated Dr. Clyde Yancy, medical director of the Baylor Heart and Vascular Institute of Baylor University Medical in Dallas, Texas. "As an unfortunate result, rehospitalization rates among African-American heart patients are high. In addition, the incidence of diabetes and hypertension in this population further complicates post-heart attack treatment. Therefore, it is critical that the African-American heart attack survivors, along with their families, are aware of the risks and gain the information and support they need."

About Heart Attacks

Each year in the U. S., nearly 900,000 people suffer from heart attacks, known medically as myocardial infarction. Within only six years, nearly 20 percent of men and 35 percent of women will have another heart attack, a risk that is heightened in the winter months. Annually, it is estimated that nearly 125,000 African-Americans will experience a new or repeat heart attack or fatal cardiovascular disease event. In fact, more than 18,000 African-American men and women died as a result a heart attack in 2002.

Heart attacks occur when the blood supply to part of the heart muscle is severely reduced or blocked. This narrowing of the coronary vessels is often linked with risk factors such as smoking, diabetes, high blood pressure, high blood cholesterol, physical inactivity and obesity. Risk factors may also be associated with family history.

About the Survey

TSC, a division of Yankelovich, conducted this online survey on behalf of the National Medical Association between August 25 and September 20, 2006 among 502 African-American adults aged 18 or older who have experienced a heart attack. The sampling error is plus or minus 4.5 percent. The survey report focuses on the findings among African-American heart attack survivors, comparing them with the general population results from a similar survey conducted in 2005 by Harris Interactive on behalf of Mended Hearts (n=518).

Lawrence A. Kotlow, D. D.S., a graduate of SUNY Buffalo, New York Dental School has been in practice since 1974 and specializes in Pediatric Dentistry in his private dental practice located in Albany, New York. The D-Carie mini is a lightweight, easy-to-use, cordless device that can be used as an aid for clinicians to quickly locate and diagnose caries. The D-Carie mini uses Light Emitting Diode (LED) and fiber optic technologies and is FDA approved as a diagnostic aid for the detection of both interproximal and occlusal caries. The device requires no calibration and is easy to sterilize.
The D-Carie mini detects more than 92 percent of occlusal caries and up to 85 percent of interproximal caries. Using only X-rays and dental explorers, dentists can overlook approximately half of incipient caries. $17 million acquisition provides license in five boroughs, Long Island and Westchester

Great Neck, NY (Vocus) August 13, 2010

North Shore University Hospital (NSUH), a member of the North Shore-LIJ Health System, has reached agreement to purchase assets of the Saint Vincent’s Catholic Medical Center Certified Home Health Agency (CHHA), subject to the approval of the United States Bankruptcy Court for the Southern District of New York. The transaction results in an eight-county CHHA license for North Shore University Hospital that includes the five boroughs of New York City, Nassau, Suffolk, and Westchester. Prior to the acquisition, North Shore-LIJ Home Care was licensed for Long Island and Queens.

The purchase includes $17 million in cash and assumption of certain obligations of the CHHA. The hospital will offer employment to those CHHA employees that meet its hiring standards and will work cooperatively with the unions to transition the work force.

“The ability to provide post acute services such as home care, in counties where we have hospitals is strategically important as we continue to bundle and integrate chronic disease across the continuum of care,” said Michael Dowling, president and chief executive officer for North Shore-LIJ Health System. “Many patients do best in their home environment. With this new acquisition we’re expanding our ability to reach the number of patients who can receive clinical services in the comfort of their own homes.”

Once approved by the bankruptcy court, the CHHA will become part of the North Shore-LIJ Health System’s Home Care Network, which provides over 800,000 home health visits annually and addresses short-term health issues such as recovery from surgery to longer-term chronic care management of conditions such as congestive heart failure, stroke, diabetes and other illnesses.

“When you look at providing service to a community – you look at how to best meet a need with quality care,” said Mark Solazzo, executive vice president and chief operating officer for North Shore-LIJ Health System. “We know that providing quality home health care is essential in our area. We believe this purchase makes sense for us and for the community from both a quality and access to care perspective.”

For depressed adolescents, Prozac plus cognitive-behavioral counseling is more effective than either treatment alone for relieving symptoms and reducing suicidal thinking, new research shows.

For depressed adolescents, Prozac plus cognitive-behavioral counseling is more effective than either treatment alone for relieving symptoms and reducing suicidal thinking, new research shows.

Based on "remarkably strong and consistent findings," Prozac is recommended for depressed adolescents, provided that they're closely monitored for side effects, according to the report in this week's Journal of the American Medical Association.

Because cognitive behavioral therapy (CBT) combined with medication is even more beneficial, increasing protection from suicidal thoughts, "CBT also should be readily available as part of comprehensive treatment for depressed adolescents," the investigators add.

The Treatment for Adolescents with Depression Study (TADS) included 439 depressed teenagers recruited at 13 clinics across the US. The subjects were randomly selected to receive Prozac, CBT, both therapies, or inactive "placebo" pills for 12 weeks, lead author Dr. John S. March, at Duke University Medical Center in Durham, North Carolina, and colleagues note.

Compared with the placebo group, only patients treated with both Prozac and CBT were more likely to experience an improvement in their depression. Although Prozac or CBT alone could improve depression, the chances of this happening were not that much different from what was seen with placebo.

The authors also found that Prozac plus CBT was better than the other treatments at reducing suicidal thoughts. Prozac alone was the least effective in this regard, but there was no evidence that the drug increased the risk of suicidal thoughts -- a topic of controversy in recent months.

However, Prozac use was associated with an increase in harm-related events, such as self-injury by cutting, aggressive thoughts, or action against another person or property.

In a related editorial, Dr. Richard M. Glass, Deputy Editor of JAMA, cautions that treatment of depression "typically requires more than a brief visit for prescription of medication."

"Rather, it requires careful assessment and monitoring in the context of an ongoing patient-physician relationship," he writes, plus "empirically validated psychotherapy."

The economic crisis is taking a significant toll on Maryland hospitals, causing their finances to slide to unprecedented low levels during the final quarter of 2008. Maryland hospitals' total margins dropped to negative 13.5 percent between October 1 and December 31, 2008, compared to 2.2 percent during the same time in 2007. The situation in Maryland is far worse than for hospitals nationwide, where the deficit for total margins was negative 7.8 percent for the final three months of 2008. Thirty-four Maryland hospitals sustained overall losses during that three-month period totaling $466 million, according to a new financial analysis by the Maryland Hospital Association.

Maryland hospitals' total margins dropped to negative 13.5 percent between October 1 and December 31, 2008, compared to 2.2 percent during the same time in 2007. Hospitals' total margins had already fallen to a negative 2.5 percent in the quarter ending September 30, 2008. Total margins measure the overall financial condition of Maryland's hospitals, which are almost exclusively not-for-profit facilities.

"At the very time when the economy is forcing more Marylanders to turn to hospitals for their only source of health care, the economy is crippling Maryland hospitals' ability to meet the growing demand," Carmela Coyle, MHA President and CEO, said. "That means hospitals have significantly less money to hire new staff, invest in new services and programs or technology, to upgrade buildings, or to help physicians."

The situation in Maryland is far worse than for hospitals nationwide, where the deficit for total margins was negative 7.8 percent for the final three months of 2008.

Thirty-four Maryland hospitals sustained overall losses during that three-month period totaling $466 million, according to a new financial analysis by the Maryland Hospital Association.

Most of hospitals' losses resulted from increases in interest rates, losses on returns, and other increases in costs needed to provide financing to upgrade or expand programs and to underwrite the costs of new technology. Operating margins--which measure financial performance in providing care--while dropping, did not fall as precipitously. Overall operating margins in the fourth quarter of 2008 were 1.5 percent--down from 2.4 percent in the quarter ending September 30. That is well below the rate-setting commission's target of 2.75 percent and below the 1.8 percent operating margins for hospitals nationwide for the quarter ending December 31.

The losses Maryland hospitals face are forcing them to limit their purchases of new technology and equipment, and to delay upgrades in information technology according to a new MHA survey. Hospitals also report adopting, or considering, hiring freezes and staff reductions; eliminating vacant positions; and cutting administrative spending, including educational and meeting expenses. In addition, hospitals report the losses have led to a significant increase in hospitals' pension liability costs.

Furthermore, the survey found the economy is forcing more and more physicians to approach hospitals for financial support, ranging from widespread requests for hospital employment to nearly universal requests for pay increases to provide services to hospitals. In 2008 hospitals provided $210 million in support to physicians.

"With a physician shortage particularly intense in inner city Baltimore and rural parts of the state, financial support for doctors is vital to assure access to care in these areas," Coyle said.

"Hospitals are the bedrocks of our communities, both as providers of health care to all who need assistance and as the economic engines for our communities," Coyle added. "Anything that weakens our hospitals poses a threat to the economy of Maryland and the well-being of our state."

MultiPlan, Inc., the nation’s oldest and largest independent PPO network, is now accepting applications for its 2010 Rural Health Outreach Grant. The program, now in its 15th year, awards ten hospitals serving rural areas grants of $3,000 each to help them introduce or expand services, education, screenings and other endeavors aimed at improving the health of people in their communities.

“The healthcare needs of the people in rural communities are as diverse as the geographic regions of our country,” said Dr. Paul Goldstein, Corporate Medical Director for MultiPlan. “From upstate New York to southern New Mexico and many places in between, these grants enable our healthcare providers to better serve the people who make their homes in the rural areas of our nation.”

The grant is available to rural hospitals participating in MultiPlan’s PHCS, MultiPlan and/or HealthEOS Networks as well as the Beech Street and Texas True Choice networks which were added to the program when MultiPlan acquired Viant in March, 2010. A new and innovative diagnostic tool designed to help pediatricians identify and rate the severity of eating disorder symptoms in children and adolescents has been pioneered by the Kartini Clinic for Disordered Eating, in Portland, Oregon, the Pacific NW's leading pediatric eating disorder practice.

The Rating of Eating Disorder Severity in Children, or REDS-C, is a semi-structured diagnostic interview originally developed by Dr. Eliot Goldner of the University of British Columbia, and adapted for use with children by Dr. Julie O'Toole, founder and medical director at Kartini Clinic. The diagnostic interview is used by pediatricians at Kartini Clinic as part of the admission process, to make more timely and accurate diagnoses of eating disorders in children.

"Pediatricians rely heavily on patients and their parents to tell them what is wrong. If parents lack knowledge about the signs of eating disorders or if they are in denial about the seriousness of their child's problems, then the doctor might miss certain vital clues to the health of a child. Detecting the vital clues of an eating disorder is what the REDS-C diagnostic tool is intended to do," explains Dr. O'Toole.

Based on the successful use of the REDS-C to detect eating disorder symptoms and rate symptom severity in over 1500 young people since 1998, Dr. O'Toole and the Kartini Clinic were invited to present REDS-C data at an international conference sponsored by the National Institute of Mental Health in March 2009. The purpose of this international conference was to develop and recommend criteria for recognizing eating disorders in children and adolescents. Kartini Clinic was selected for participation in this distinguished conference because of its specialization in diagnosing and treating young children with eating disorders and Kartini Clinic's reputation for successfully treating children and adolescents with the most severe and treatment resistant disorders.

Researchers from Kartini Clinic, the University of Rochester, and the University of North Dakota continue to study the REDS-C. According to Dr. DeSocio, researcher from the University of Rochester, "The REDS-C is a valuable tool because it allows clinicians to rate the severity of eating disorder symptoms on 16 dimensions. The child's score on individual items allows the clinician to distinguish the type of eating disorder a child has and the severity of the child's illness. Clinicians can use these findings to make decisions about the best treatment for the child. The REDS-C is also a useful tool to evaluate change in the child's symptoms as a result of treatment. These unique features of the REDS-C make a much needed contribution to the treatment of our youngest patients with these devastating disorders."

Dr. O'Toole reflects on the value of the REDS-C in her years of clinical experience with children: "We believe that early intervention in children, especially for anorexia nervosa, can greatly improve prognosis and lead to higher rates of successful treatment and lasting recovery. Any diagnostic tool that can assist in catching this illness in its earliest stages has the potential to improve treatment outcomes significantly."

Results of research on the REDS-C are currently being reviewed for publication in a peer-reviewed professional journal.
