Statement of Need:

Schizophrenia is notable for its substantial impact on everyday functioning, being one of the top five causes of disability in individuals under the age of 25. In most cases, individuals who develop schizophrenia manifest behavioral changes prior to the formal diagnosis of the condition. These changes, referred to as the "prodrome," are variable across individuals who may eventually develop schizophrenia. Proactive risk assessment in other chronic diseases such as cardiovascular disease, diabetes mellitus, and cancer has brought about advances in early detection and intervention leading to substantial reductions in morbidity, mortality, and improved quality of life. It is imperative that this type of proactive intervention becomes part of timely diagnosis and better outcomes for patients with schizophrenia.

Clinicians face challenges designing treatment plans for patients with schizophrenia that will maximize adherence and reduce side effects. Relapse rates tend to be quite predictable, can be affected by adherence, and have multiple adverse consequences. Minimizing relapse requires a coordinated effort between the patient, provider, family, and multidisciplinary treatment team. In this interactive neuroscienceCME activity, faculty will focus on the impact of treatment selection, phase of illness, medication adherence, and side effect burden on the journey toward remission and recovery in schizophrenia.

"I remember waking up in the recovery room screaming in pain," said patient Kristina Henry of her six-hour surgery, followed by a month's recuperation. Describing one physician's approach she said, "He thought I was going to go straight from surgery and start having sexual intercourse, so he took out my stent that was supposed to stay in, according to my original doctor. I was supposed to dilate and at 16 that was not a top priority for me. It hurt and I was so tired of pain. After time this made my length decrease and eventually it decreased to barely nothing. To this day I cannot wear a swimsuit without a cover-up because my scars are very noticeable."

Problem Often Undetected Until Puberty

Up to a half million women in the U. S. are born with anomalies of the urinary and reproductive system. In females thousands of these mullerian anomalies, including the absence of a vagina, may not be discovered until the girl reaches puberty.

MRKH (Mayer-Rokitansky-Kuster-Hauser) syndrome includes failure of the vaginal development, whether or not the uterus is present. Adolescent girls with MRKH syndrome and obstruction defects may complain that they have not begun menstruation, labeled primary amenorrhea.

In some cases, female patients have discovered their anomaly after inability to have intercourse because of an absent vagina. For many, multiple painful attempts of intercourse have led to distortion of existing external genital structures and scarring of the area, as well as chronic pelvic pain.

Laparoscopy for Vaginal Aplasia, or Non-Development of the Vagina

Techniques for correction over the years involved use of segments of the rectum, sigmoid colon and small intestine, as well as skin graft. In 1898, using open surgery, Dr. D. O. Ott formed a neovagina using peritoneum of the pelvis. It was then discovered that the cells of the peritoneum (the smooth transparent membrane lining the abdominal cavity) rapidly converted into typical vaginal cells. Different approaches to this technique evolved over time.

Nearly 100 years later, in 1993 Dr. L. V. Adamyan developed a laparoscopy-assisted technique for colpopoesis (creation of the neovagina), using the pelvic peritoneum. This minimally invasive technique is far easier on the patient, as it involves only tiny incisions and is performed in less than an hour. More than one thousand of them have been performed, proving their efficacy.

The procedure allows achievement of adequate functional vaginal length and elasticity, and the peritoneal epithelium (cellular layer that lines the walls of the abdominal cavity) converts into normal vaginal epithelium within three months after surgery.

World-renowned head of the Center for Women's Care & Reproductive Surgery, Thomas L. Lyons, M. D., and his Fellow, Assia A. Stepanian, M. D., learned the technique firsthand from its author, and they are the only U. S. surgeons with experience in laparoscopic correction of MRKH syndrome using the pelvic peritoneum.

This procedure was used to correct Kristina's post-surgical shortening of the vagina, which occurred from previous surgeries.

"It's very unfortunate that so many women have these suboptimal procedures because they often fail," said Dr. Lyons about the old style MacIndoe split thickness skin graft, which Kristina endured originally.

Blessed with a loving husband who told her he didn't see her scars, Kristina wanted to have a normal sex life but had lost most of her depth.

According to a recently released NHTSA survey, 20 percent of drivers 16 and older say they have driven after drinking, which puts other motorists at risk of injury or death, Rhode Island personal injury attorney Mark Gemma says.

Rhode Island personal injury attorney Mark Gemma (http://www. gemmalaw. com/mark-gemma. asp) says he is troubled by a recently released National Highway Traffic Safety Administration survey showing a high number of drivers who admit to driving after drinking.

In the survey, 1 in 5 drivers ages 16 and older said they had, during the past year, driven within two hours of drinking alcohol. About two-thirds of those drivers, or 13 percent of the total population, said they had done so within the past 30 days.

According to NHTSA, those results meant that an estimated 85.5 million drinking-and-driving trips had occurred nationally in the past month, an increase from the 73.7 million reported in 2004.

“These survey results illustrate the need to take tougher measures against drunk drivers and raise more awareness about the human, criminal and civil consequences of drinking and driving,” says Gemma, whose law firm, Gemma Law Associates, Inc., represents the victims of DUI / DWI accidents (http://www. gemmalaw. com/drunk-driving-accidents. asp) and their families throughout the state of Rhode Island.

“It’s alarming to me that more people may be drinking and getting behind the wheel of their cars, especially in a state such as Rhode Island, which has one of the highest drunk driving fatality rates in the nation.”

According to NHTSA statistics from 2008, the latest year for which numbers are available, 38 percent of the state’s motor vehicle fatalities involved a driver with a blood alcohol concentration (BAC) of.08 or higher. The state ranked fifth in the country in that category.

As recently as 2004, Rhode Island ranked first in the country in the percentage of traffic fatalities related to drunk driving.

“Drunk driving accidents turn lives upside down for victims and their families, and the drunk driver faces criminal penalties and a potentially significant personal injury or wrongful death lawsuit,” Gemma says.

“A drunk driver could be held liable for a variety of damages, including medical expenses, funeral costs, lost wages, disfigurement, disability, rehabilitation, pain and suffering, emotional distress and the loss of the services and companionship of a loved one,” Gemma says. “Punitive damages are also frequently awarded in DUI/DWI accidents.”

In addition to drunk driving, statistics also show that an alarming number of Rhode Island drivers get behind the wheel after using illegal drugs, Gemma points out.

According to a 2008 study by the Federal Substance Abuse and Mental Health Services Administration, nearly 7 percent of Rhode Island adult drivers had operated their vehicles in the prior year under the influence of illicit drugs.

“When it comes to holding a personal accountable for the bodily injury and property damage they have caused in a car accident, there’s little difference between a drunk or drugged driver,” Gemma says. “The bottom line is that impaired driving is dangerous driving.”

Instead of driving after drinking, Gemma suggests that Rhode Island residents call a taxi, designate a sober driver, use public transportation or call a friend or relative for a ride home.

Another contract was announced in April in which Bravo! announced that they would Launch General Mills Branded Flavored Milks including TRIX, COCOA PUFFS, and WHEATIES flavored, fortified, extended shelf life milk drinks under a recently signed five year trademark licensing agreement with General Mills Marketing, Inc. In addition to these brands the agreement covers LUCKY CHARMS, COUNT CHOCULA, BOOBERRY, and FRANKENBERRY. Bravo! has the right to use the equity characters associated with each brand.

In May, Bravo announced that it had signed two new seven year licensing agreements for Canada and Mexico with Masterfoods USA, a division of Mars Incorporated. Kids Tout 'Super Hero' Savings : America's youth reports strong savings habits

More kids living in the U. S. associate the state of their piggy banks with “super hero” strength and stability than with Halloween gloom and doom, according to a new poll conducted by the financial literacy Web site www. themint. org. The finding provides parents and teachers with an optimistic glimpse into the future, even as difficult economic conditions continue to grip the nation.

In the survey, visitors to the site were asked to choose which Halloween costume best describes the state of their piggy banks. A majority of the respondents 17 years or younger (56 percent) said they either had strong, unshakeable "super hero" savings, or savings that evoked a "train engineer" costume, showing their plans for savings are right on track. This is good news because only 26 percent of respondents described their savings as "bare bones" or downright laughable.

"It's encouraging that kids see the benefits of savings at a young age," said Meridee Maynard, senior vice president, Northwestern Mutual. "The power of saving for the future may not ever allow anyone to leap buildings in a single bound, but it can help people achieve feats far more important -- like attending college, buying a home, and retiring comfortably."

When asked what they would do if handed a sum of money, more kids preferred saving over spending. More than half of the kids aged 17 years or younger (54 percent) said they'd save all of their money and/or budget it carefully and slowly, compared to a small number (15 percent) who said they'd rather spend it as fast as possible.

From July through September 2008, visitors to the financial literacy Web site www. themint. org were invited to answer two questions related to individual savings habits. A combined total of more than 1,600 respondents from across the country weighed in about what they would do if handed a sum of money, as well as the state of their current savings. The results were then analyzed based on several demographic factors, including the age, gender and location of respondents.

Males Report Least Desire to Save, Strongest Savings

While kids as a whole seem to understand the importance of saving and budgeting in a healthy financial plan, the poll revealed some surprising findings when the demographic is broken down by gender and geographic regions.

According to the survey, young male respondents are more confident with their savings, but less concerned with budgeting than their female counterparts. While less than half (47 percent) of the males aged 17 or younger said they would save their money or budget it carefully, an overwhelming majority (87 percent) saw their savings as strong or on track. Conversely, more females (59 percent) said they would be vigilant with the money, but a significantly smaller number (50 percent) are confident that their savings are on course.

Westerners Exhibit Good Intentions but Lack Confidence In Their Savings

Interestingly, where kids live appears to affect their attitude toward saving and financial confidence. Two-thirds (67 percent) of respondents in the West said they would look after their money conservatively, but less than half (48 percent) are satisfied with the strength of their actual savings.

The rest of the country was far more consistent. Over half of the kids in the Midwest and Northeast said they would be diligent with the money and are confident in their savings, while slightly less than half of Southerners said they were content with the state of their piggy bank.

This study is part of an ongoing series conducted on www. themint. org. Polls released in April and July 2008 focused on allowances and summer employment, respectively.

About the Clinical Trial:
The clinical trial will be conducted under the direction of Prof. Anat Loewenstein MD, who is an associate Professor of Ophthalmology & the Vice-Dean of the Sackler Faculty of Medicine at the Tel Aviv University, and the Director of the Department of Ophthalmology at the Tel Aviv Medical Center in Israel. The prospective study, which includes 102 patients (102 eyes), was designed to evaluate the effectiveness of Lumenis Selective Retina Therapy (SRT) treatment in diabetic macular edema. The duration of the study is two years with patient follow up at 4, 8, and 12 months post-op.
All patients will receive treatments with the Lumenis SRT laser that was developed by Lumenis Ltd., in collaboration with its research partner MLL (Medizinisches Laserzentrum Lübeck) in Lübeck, Germany. Lumenis holds propriety rights for this technology.

About Selective Retina Therapy (SRT):
Selective Retina Therapy (SRT) is a relatively new laser technique which selectively targets the Retinal Pigment Epithelium (RPE) while sparing the neural retina. Several macular diseases are thought to be caused and/or significantly exacerbated due to reduced function of the RPE cells. In light of that, a method for the selective destruction of the underperforming RPE cells without causing adverse effects to the choroid and neurosensory retina (especially to the photoreceptors layers) is hypothesized to halt the progression of those diseases and/or reverse some of its deleterious effects.

The selective effect on RPE cells, which absorb about 50% of the incident light due to their high melanosome content, has been previously demonstrated using the Lumenis SRT laser. By irradiating the fundus with a train of 1.7 micro-second laser pulses it is possible to achieve high peak temperatures around the melanosomes, which leads to a destruction of the RPE, with only a low sub-lethal temperature increase in adjacent tissue structures. This process leads to the multiplication and migration of healthy RPE cells from the periphery which, in turn, help metabolize and improve the overall retina tissue health.

The Lumenis SRT laser was specifically designed to perform selective RPE targeting. This is accomplished by a green (frequency doubled Nd:YLF, 527nm) laser that emits 1.7 microsecond (¼s) pulses at a repetition rate of 100 Hertz (Hz). This low average power, delivered in a train of pulses, confines the laser treatment to the cells containing the target pigment or chromophores, in this case the melanin granules in the RPE cells, thereby achieving selective damage to these particular cells.

The United Nations Foundation joins the international community in expressing its deep disappointment that the administration has decided — for the seventh, straight year — to withhold the $39.7 million authorized by Congress to the United Nations Population Fund.

"The United Nations Foundation joins the international community in expressing its deep disappointment that the administration has decided — for the seventh, straight year — to withhold the $39.7 million authorized by Congress to the United Nations Population Fund (UNFPA), the world’s leading voice on sexual and reproductive health and rights.

“In a statement notifying Congress of the administration’s decision to withhold funds from UNFPA, Deputy Secretary of State John Negroponte once again cited UNFPA’s program in China as a violation of the Kemp-Kasten Amendment, which bars funding for programming that 'supports or participates in the management of a program of coercive or involuntary sterilization.'

“UNFPA does not — and has never — supported coercive or involuntary sterilization. In fact, the decision to withhold funds from UNFPA is inconsistent with the reports from the State Department and several other blue-ribbon investigative teams, which included descriptions of UNFPA’s work as 'a force for good' in China.

“Working in 150 countries, UNFPA is on the front lines reducing maternal and infant mortality, decreasing HIV/AIDS rates, and protecting women and girls from rape and violence, particularly during conflict situations. The $34 million that the United States has withheld each year is close to 10 percent of UNFPA’s regular income. The amount withheld every year could have helped UNFPA prevent 2 million unintended pregnancies, 800,000 abortions, 4,700 mothers’ deaths, and more than 77,000 infant and child deaths. Approximately 181 industrialized and developing countries, including all the countries in sub-Saharan Africa and Latin America, contribute to UNFPA. The United States is the only country to withhold funding for political reasons.

“The UN Foundation is looking forward to working with the next administration to restore funding for UNFPA and to strengthen the U. S.’s role as a global health leader. During the 2000 UN Millennium Summit, the United States pledged to work to respond to the world's most pressing development challenges, including poverty, gender inequality and disease. It is past time that the administration acknowledges how fundamental UNFPA is to addressing these global challenges and that the U. S. funds UNFPA’s work."

Over the past 30 years the number of single-mother families has increased 333%; single-father families have grown 508%. Seven out of ten households with children are maintained solely on the income of one parent. As you can imagine it gets harder everyday to meet the demands of parenting.
