Efficient, effective and reliable remote healthcare monitoring is a holy grail in medicine but solutions have so far proved elusive. But it took a step closer to reality with the successful conclusion of pilot tests under the E-Care project.

E-Care developed a comprehensive monitoring system to capture, transmit and distribute vital health data to doctors, carers and family. Pilot tests of the system indicate that doctors, nurses, patients and their families found E-Care reliable, simple to use and an effective method to improve the quality of care while reducing costs.

It monitors patients with chronic, or long-term, illnesses such as diabetes or cardiovascular disease, and patients discharged after an operation or serious medical crises, such as stroke victims. It can acquire vital information about a patient who lives far away from medical support, and it can alert medical staff if there is a dangerous change in patient's status. With e-CareÂ’s system doctors spend less time going to see patients and more time treating them. It also means real-time monitoring without high staff or capital costs.

"Citizens with long-term illnesses as well as those who are in post-surgery state, or predisposed to illness, need monitoring of their health until their condition becomes stable," says project director Mariella Devoti. "They, as well as their family and friends, also need an efficient way to collaborate with their doctor and get informed about their state. Until now, monitoring of the health condition of such people could only be accomplished by prearranged visits from a doctor, or by visits to the local hospital for a check-up. However, this is an inefficient solution, as well as costly, as these visits would scarcely be on a daily basis."

E-CareÂ’s remote monitoring system includes a wireless intelligent sensor network (WISE), bio-medical sensors and a radio terminal. WISE consists of a series of monitors that track signs like activity, temperature, pulse, blood pressure and glucose or other personal data like weight, pain measurement and drug conformance. Data collected by the sensors are sent to the transmitter that sends them to the central system.

SMS messages sent to the drug conformance device remind the patients when they need to take medication. Patients send a confirmation once they take the drugs.

The central system includes a medical data manager (MDM) that automatically checks patient data against the patient's record and any doctor's notes. If there is a disturbing change in the patient's vital signs, for example high glucose levels in a diabetic, an alarm is sent directly to the patient's physician. This provides peace of mind for patient and family and ensures a doctor or medics can respond rapidly to any problems that arise. Similarly, the MDM can alert paramedical staff or a doctor if patient data fails to arrive when expected.

The E-Care repository stores all patient data. The collaboration module allows user to communicate using real-time synchronous message, audio conferencing or videoconference. Patients and family can confer with their doctor, or GPs or nurses can confer with a specialist.

The workflow system controls overall system processes, while user Web applications dynamically format and present the patient data depending on who is accessing the information and what access rights they have.

Researchers used standard and widely adopted technologies where possible, so the system will work with any modern hospital or clinic.

E-Care deployed in three pilot programmes between February 2003 and February 2004. "The final validation results for E-Care showed a general satisfaction among doctors and medical staff, system and medical administrators, patients and their family," says coordinator Devoti.

Users praised its practicability, reliability, effectiveness and patient acceptance.

Diabetes specialists praised the glucose monitoring system. It allows the doctor to change patient treatment based on vital sign analysis. Similarly the blood pressure monitor was particularly of interest for patients with stroke or cardiovascular disease.

With the project validation complete the partners will fine tune the system and search for commercial opportunities. Remote monitoring is now not so distant.

Since a Fugu cosmid clone typically contains 6 to 8 genes and a BAC clone contains up to 15 genes, Fugu genomic clones are ideally suited for dissecting gene regulatory regions and locus control elements. • Turn off the Water – Turn off the water when you brush your teeth or shave, and fix those leaky faucets.

• Clean Up a Stretch of a Local Stream - Visit the nearest body of water, lake, stream, ocean, or estuary. Look for and catalog evidence of human use and occupation. Is there garbage in or near the water? Sort and classify the garbage. Is the water discolored or spotted with foam? Is there visible life in the water or at the edge? Make a survey of the local fish markets to find out where their fish comes from. Ask about the quality of the local bodies of water.

• Install a low-flow, energy efficient showerhead. YouÂ’ll be singing in the shower, “We're in the Money," as a low-flow, energy-efficient showerhead saved 27 cents a day on water and 51 cents on electricity. Earth Day can be the day to install low-flow toilets, shower-heads, and faucet aerators.

• Contact your local recycling office to find out how to dispose of nasty chemicals that can pollute the water supply. Paints, solvents, pesticides, wood finishers, and non-biodegradable household cleaners are chemicals that many people have around their homes. If disposed of improperly, they can seep into the ground or enter the water supply. Even batteries are harmful and toxic.

• Monitor lawn and garden care. Fertilizers and pesticides can seep into the ground, or run off into a near by water supply. Also, donÂ’t waste water and kill plants by over watering gardens and lawns, and donÂ’t water mid-day when the sun will evaporate the water your lawn needs.

Beginning this year, Earth Day NetworkÂ’s new goal is to provide educational materials for students to take civic action to protect the environment. April 22nd is an opportunity to highlight student environmental efforts, integrate service learning, and encourage civic participation. Earth Day Network is providing, a "WhatÂ’s in Your Water?" guide. In the guide, there is basic information about water quality and lesson plans with simple, fun hands-on water quality tests. International Medical Clinics (IMC) is seeking men and women age 50 and older who have had a heart attack to join a National Institutes of Health (NIH) study on the effectiveness and safety of an investigational treatment for heart disease. The investigational treatment is a combination of chelation therapy and high-dose vitamin therapy compared with placebo (inactive treatment).

International Medical Clinics (IMC) is one of more than 100 medical institutions across the country selected to take part in the Chelation Therapy Study, the largest clinical research study to evaluate the therapy's effectiveness and safety as a treatment for heart disease.

Chelation (pronounced key-LAY-shun) therapy is an investigational therapy using a man-made amino acid, called EDTA (ethylene diamine tetra-acetic acid). It is added to the blood through a vein. A national research study is now testing whether chelation therapy is safe and effective for treating heart disease.

“Many people are considering using chelation therapy because of the belief that it may treat heart disease. However, this has not been scientifically proven. Past studies did not show benefit, but may have been too small to be conclusive," said Dr. Lisa Merritt. "We are committed to working with the NIH to find the answer to this important public health question."

The study is more than 20 times larger than any previous study of chelation therapy and is designed to be large enough to detect even moderate benefits associated with this therapy.

Coronary artery disease – the most common form of heart disease – is the leading cause of death of Americans, affecting more than 7 million people and killing 500,000 people each year. It occurs when the arteries that supply blood to the heart muscle (coronary arteries) become hardened and narrowed. The arteries harden and become narrow due to the buildup of fatty materials like cholesterol, called plaque, on the inner walls or lining of the arteries (atherosclerosis). Blood flow to the heart is reduced as plaque narrows the coronary arteries. This decreases the oxygen supply to the heart muscle. In a heart attack, a blood clot forms on the plaque and completely blocks flow through the artery.

Study participants will be assigned randomly (by chance, like flipping a coin) to receive a standardized chelation solution or a placebo (inactive) solution over a period of 28 months. Because chelation therapy may also remove important vitamins and minerals the body needs, all study participants will take low-dose vitamin and mineral supplements. In addition, some participants will receive a high-dose vitamin and mineral supplement. The study will also evaluate whether high-dose vitamins and minerals are helpful. Neither the participants nor the researchers administering the treatment will know which of the four treatment combinations is being given. Using this process to assign people to treatment groups means that the groups will be as similar as possible and that the treatments they receive can be compared objectively.

Those who join the study will be part of a nationwide effort to help the medical community find new and effective treatments for heart disease. In addition, their health will be closely monitored while they are enrolled in the study.

There are some potential risks and side effects to chelation therapy. The most common side effect is a burning sensation at the site where the EDTA is delivered into the veins. Rare side effects can include fever, headache, nausea, and vomiting. Even more rare are serious side effects that can include a sudden drop in blood pressure, abnormally low calcium levels in the blood, permanent kidney damage, and bone marrow depression (meaning that blood cell counts fall). Reversible injury to the kidneys, although infrequent, has been reported with EDTA chelation therapy. Other serious side effects can occur if EDTA is not administered by a trained health professional.
